As a key people leader within the service, you will support the operational running of our Day and Night Nursing Team, ensuring patients and their families receive the best possible support.

About us

Hospice at Homes core service is our skilled team of Registered Nurses and Healthcare Assistants who provide exceptional care and support in people's last year of life, at end of life, also providing additional services such as Lymphoedema, Bereavement and Family Support, Befriending, Occupational Therapy and Complementary Therapy.

We provide individualised care and support to improve quality of life and our end‑of‑life care helps facilitate a peaceful and dignified death in the home and support to those who are bereaved.

Our clinical service area covers a mixture of urban and rural communities, a region of approximately 1,500 square miles and a population of 200,000 throughout North and East Cumbria.

Job responsibilities

To work in a specialised Hospice at Home team as a Registered Nurse, without direct supervision, to lead on the assessment and re‑assessment of patients and carers receiving home nursing and volunteer support from Hospice at Home Carlisle and North Lakeland.

To coordinate and manage the Hospice at Home Day Night Nursing Team, acting as Line Manager and co‑ordinator for the Day Night Nursing Service in the absence of the Head of Clinical Services.

Assist the Head of Clinical Services to develop and support nurses and the wider team through an effective appraisal and development review process, identifying learning needs and providing training opportunities.

Liaise with the Head of Clinical Services as appropriate to review care provision.

Participate in indirect provision of patient care and liaise with members of the multi‑disciplinary team and palliative care team in order to achieve this.

Person Specification Qualifications
  • Registered Nurse
  • Palliative Care Diploma
Skills
  • Ability to work as an autonomous practitioner and as part of a team.
  • Enhanced communication skills to work with patients, carers and health care professionals.
  • Enhanced care delivery skills with the ability to multi‑task and manage a busy and varied workload, working methodically and paying
Knowledge
  • Wide knowledge of medical conditions and symptom management.
  • Knowledge of audit.
  • Knowledge of current palliative care agenda both nationally and locally.
  • In‑depth understanding of multi‑disciplinary issues and commitment to inter‑disciplinary working.
  • Knowledge of third sector organisations within service area.
  • Knowledge of non‑malignant conditions.
Experience
  • Proven post registration experience and experience of working effectively in a team.
  • Working with palliative care patients and families.
  • Experience in Appraisal process and the training, support and development of staff.
  • Providing clinical supervision to others.
  • Community working.
  • Experience of Line manager others
