Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Active Kentucky Physical Therapist license required to start.
• Physical Therapist credential verification required to start.
• CPR (Basic) required to start.
• HEP B required to start.
• MMR required to start.
• VARICELLA required to start.
• TB required to start.
• Tdap required to start.

• Shift schedule: 5x8.
• Shift hours: 12:00 AM - 12:00 AM.
