Reporting

& Team

Reporting to a Director of Population Health/Care Management with close partnership to Quality, Informatics, and Finance, the Nurse Manager Population Health will lead friendly nurses and a diverse team of RN care coordinators, social workers, community health workers, pharmacists, and analysts.
