Job Description- Administer skilled nursing care for clients of all ages in their place of residence.
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to support staff to ensure quality service.
- Monitor assigned cases for compliance with third-party payer requirements.
- Provide effective communication with patients, families, and healthcare professionals.
- Flexible schedule with on‑call responsibilities, covering Boone, Kenton, Campbell, Grant and Pendleton Counties (Northern KY).
- Current state license as a Registered Nurse.
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- CHPN certification preferred.
- Proof of current CPR certification.
- Benefits are available to full‑time employees after 90 days of employment.
- A 401(k) with company match is available after 1 year of service.
