This practice sees patients in skilled nursing facilities, assisted living facilities, independent living facilities, and private homes. The provider will work with the practice to establish a mutually agreeable geographic territory for patients.
Company ProfileThis organization delivers convenient, high‑quality primary care to homebound and facility‑based geriatric patients. The team currently includes more than 15 physicians and nurse practitioners who provide care without the need for patients to travel. Services include primary care, chronic care, psychiatric care, post‑acute care, transitional care, and remote patient monitoring.
Position DescriptionThe nurse practitioner serves as the primary care provider for a dedicated panel of geriatric patients located in facilities or private homes.
On SNF days, providers see an average of 15 to 16 patients in a single location with no travel. On travel days, providers see 9 to 12 patients per day, adjusted for travel time.
Specific duties include, but are not limited to:
- Conduct health assessments to identify patient needs
- Develop personalized care plans
- Regularly evaluate patient conditions and adjust treatment plans as necessary
- Administer medications, treatments, and other therapeutic interventions as prescribed
- Perform minor procedures and wound care as required and as settings permit
- Monitor vital signs and patient responses to treatment
- Educate patients and families on health management, disease prevention, and treatments
- Provide guidance on lifestyle modifications and self‑care practices
- Work with physicians, nurses, and other professionals to coordinate comprehensive care
- Ensure compliance with all relevant healthcare regulations and standards

Works EHR.
On‑call is phone triage only with very low volume. Providers rotate the on‑call responsibility, with a shift every 10 weeks or less.
Requirements- Ohio NP license
- DEA registration
- At least 1 year of advanced practice experience
- Previous experience in geriatrics, LTC, SNFs, house calls, or related settings
