The Case Manager (CM) coordinates inpatient rehabilitation care for a designated case load of patients and refers patients for post inpatient care. CM collaborates with all team members for identification of patient/family needs/goals, achieving economical utilization of services and planning discharge as well as follow-up care with the patient, family/caregiver, and other facility. The Case Manager serves as the IRF PAI Coordinator for their group of inpatients.

Monday - Friday, 8:00am - 4:30pm

• Current Mississippi Registered Nurse License
• BSN, MS in Nursing and CRRN preferred
• Evidence of strong interpersonal skills, verbal and written communication skills
• Ability to physically assess and/or analyze patients with acute or rehabilitative requirements
• Demonstrates proficiency with utilizing standards of care, competency with nursing process and FIM scoring
• Knowledge of third-party reimbursement systems and utilization of referral services or ability to acquire knowledge
