Job Description:
Manage a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care. Ensure optimum utilization of the patient's and the Health System's resources and perform these duties within the requirements of CMS and other external review agencies. Manage, facilitate, and coordinate the necessary care activities and treatments, applying an approach to care delivery that is called the case management process.
Manage the communication among the varied care providers and other essential parties internal and external to the health care organization. Identify patients who would benefit from case management services, and the activities of assessment, problem identification, care planning, care delivery, monitoring, and evaluation of the care provided, specifically for its relevance to the needs of the patient/family, and for the health care team’s ability to meet the desired outcomes and established goals.
For those Case Manager RNs working with heart failure patients, the following applies:
This position will solely focus on hospitalized patients being treated for heart failure. The purpose of disease-specific case management is patient education and optimal discharge planning with the goal of improving patient success at home thus preventing readmission and reducing mortality. This position will work closely with the Transition of Care Pharmacist, Heart Failure Clinic, and Remote Patient Monitoring program as well as nursing and medical providers.
Bachelor of Science in Nursing (BSN) degree from an accredited institution; a combination of education and clinical experience may be considered in lieu of bachelor’s degree. Current unrestricted licensure as a Registered Nurse in the state of Kentucky required. Three years of job-related experience required. Case Management certification (ANCC, CCM or ACM-RN) or other approved Professional Certification preferred upon hire, required within two years of hire date.
Preferred Qualifications:Previous Case Management experience preferred.
