examples of duties or responsibilities of the classification

conducts comprehensive initial nursing assessments; determines program eligibility, needed services and payer source. Develops an individualized plan of care in consultation with the client’s physician and other appropriate agency personnel. Completes review of medications; assesses pain management and need for health care equipment/technology. Provides intermittent skilled nursing services, as ordered by the physician. Obtains specimens, as ordered, and submits to state lab utilizing appropriate laboratory services guidelines.

Identifies health risks and implements risk reduction strategies for self and others according to agency policy. Conducts or participates in team meetings/case conferences to evaluate client’s response to care; alters the care plan; and makes referrals to other community agencies. Develops/conducts educational counseling/teaching activities for the individual clients and families. Provides thorough documentation and submits appropriate reports, as required. Provides increasing leadership and supervision;

participates in utilization review for quality of services; may mentor lhn i hh nurses.
