Registered Nurse – Home Health | Elizabethtown, KY | 13 Weeks Contract

We’re seeking an experienced Registered Nurse – Home Health to join a dedicated home healthcare team in Elizabethtown, KY.

Roles and Responsibilities
  • Provide skilled nursing care to patients in their homes based on individualized care plans and physician orders
  • Perform comprehensive initial and ongoing patient assessments
  • Monitor patients' health status and identify changes that require intervention or escalation
  • Develop, implement, and evaluate individualized home health care plans
  • Administer medications, injections, IV therapies, wound care, and other treatments within scope of practice
  • Provide wound care and monitor healing progress, including surgical wounds, pressure injuries, and chronic wounds
  • Educate patients and caregivers on medications, disease management, treatment plans, and self-care techniques
  • Assess patients' home environments and identify potential safety risks
  • Coordinate care with physicians, therapists, social workers, case managers, and other members of the healthcare team
  • Communicate changes in patient condition promptly to physicians and appropriate clinical leadership
  • Maintain accurate and timely documentation in the EMR in accordance with home health requirements
  • Participate in care coordination, case conferences, and discharge planning
  • Provide education on fall prevention, infection prevention, medication management, and disease-specific care
  • Follow Medicare, state, agency, HIPAA, infection-control, and patient-safety requirements
  • Build trusting relationships with patients and families while maintaining professional boundaries
Qualifications
  • Active Kentucky Registered Nurse (RN) License
  • Minimum 1 year of recent Home Health nursing experience
  • Current BLS Certification required
  • Home Health or OASIS experience preferred
  • Strong knowledge of skilled nursing, wound care, medication management, and chronic disease management
  • Familiarity with home health EMR systems and clinical documentation
  • Excellent assessment, communication, organizational, and critical-thinking skills
  • Ability to work independently and manage a daily patient schedule effectively
  • Valid driver's license and reliable transportation may be required for home visits
  • Strong commitment to patient-centered care and clinical excellence
