The Street Outreach Service Coordinator is a full‑time, direct service role dedicated to engaging and building trust with individuals and families experiencing homelessness on the streets or in encampments. This position plays a key role in helping participants break the cycle of homelessness by facilitating the transition from the streets to interim housing, connecting them with essential social services, and supporting them in securing permanent housing as quickly as possible.
Utilizing evidence‑based practices, this position will engage and motivate participants, focusing on a housing‑first approach to service planning. Duties are carried out in alignment with Welcome House’s Core Values:

Duties & Responsibilities Outreach
- Identify and provide outreach services to those experiencing homelessness in the community, on the streets and in encampments.
- Providing essential services necessary to reach out to unsheltered individuals and families; connect them with emergency shelter, housing, or critical services.
- Provide transportation for program participants and their families, as needed.
- Work closely with the street outreach team lead and supervisor of regional services to prepare a schedule to ensure all counties are covered.
- Responsible for providing documentation of homelessness.
- Collaborate with the Outreach Team to assist program participants in accessing resources.
- Work cohesively with other outreach community members and the FIVCO CoC Community.
- Identify and engage needed partnerships that will improve services and/or processes, and reduce barriers for program participants.
- Provide respectful and collaborative support to service team members and share responsibility with all team members for program participant outcomes.
- Represent the agency, Outreach team and/or Supervisor of Regional Services at outside agency meetings on an as needed basis.
- Maintain positive public relations and serve as a liaison with other related community services.
- Maintain up to date program participant list.
- Keep accurate data on all program participants and assist the Supervisor of Regional Services with data gathering and reporting, as needed.
- Provide information to the Supervisor and/or Director of Regional Services for quarterly and annual reporting purposes.
- Provide analysis of data and outcomes to the Supervisor of Regional Services, when requested.
- Perform any other duties specified by the Supervisor of Regional Services or Director of Regional Services.
Education
- Minimum Associate Degree to Bachelor’s Degree or equivalent
- Chemical dependency certification, preferred
Experience
- A minimum of 1-3 years’ experience in the social service field
Skills/Specialized Knowledge/Abilities

- Provide training & mentoring of case managers
- Advanced knowledge of job readiness

- Participating in leadership responsibilities within case management team
- Involvement in and understanding of quarterly and annual reports
- Meet departmental compliance requirements with very limited supervision
- Conduct outreach to identify and engage target population
