Job Description and Duties

Effective July 1, 2025, in accordance with the applicable Memorandum of Understanding, the Personal Leave Program 2025 (PLP 2025) was implemented. PLP 2025 requires each full-time employee in Bargaining Unit 17, and related Excluded, Exempt and Statutory Exempt employees, will have their base salary reduced by 3 percent and will receive 5 PLP 2025 leave credits monthly through June 2027. Salaries do not reflect the recent changes.

Part-time employees are subject to the same conditions as full-time employees, on a prorated basis equivalent to their time-base. Permanent intermittent employees are subject to the same condition as full-time employees, based on the number of hours worked in the pay period.

Under the direct supervision of the Supervising Registered Nurse (SRN) III, the Case Manager Throughput (CMT) is a facility-based role responsible for the real-time coordination and oversight of patient movement across levels of care within the institution. This position serves as the central point of contact for ensuring timely admissions, transitions, and discharges across intermediate and high-acuity care settings such as the Correctional Treatment Center (CTC), Outpatient Housing Units (OHUs) and specialized units such as Assisted Living OHU, Memory Care Unit, Palliative Care, Hospice, Geriatric Care Management, and general population housing.

The CMT collaborates with interdisciplinary teams-including Nursing, Medical, Mental Health, Custody, and Utilization Management-to ensure that patient placement aligns with clinical needs, regulatory requirements, and operational capacity. This position is essential in supporting optimal patient flow, reducing inappropriate bed utilization, and enhancing continuity of care.

The CMT will collaborate with health care providers and other members of the interdisciplinary team to ensure patient placement decisions reflect clinical severity, nursing resource needs, and patient care requirements. The CMT will provide nursing care within the scope of practice as outlined by the Nurse Practice Act and the California Board of Registered Nursing and in compliance with Titles 15 and 22, the Department Operations Manual, the Health Care Department Operations Manual, all lawsuit directives, and all other related guidelines to ensure quality and continuity of care is delivered.

The CMT is also required to attend, and successfully complete required trainings as directed and maintain current required licensure and certifications.

Minimum Qualifications

Possession of a current license as Registered Nurse in California. (Applicants who do not meet this requirement will be admitted to the examination, but they must secure the required license before they will be considered eligible for appointment.)
