Clinical Nurse Coordinator (TCU)

Location: Guardian Angels Care Center – Elk River, MN
Salary Range: $85,925 to $119,617 (Based on Experience)

Position Summary:
In this role, you will manage the clinical care of residents on a TCU rehabilitation stay via individualized care planning, performing resident, family and staff education, delegating tasks to nurses within the department in a high acuity setting and assisting with discharge planning. Job Duties:

• Work to coordinate, oversee, and provide resident-related care.
• Manage and delegate tasks to nursing staff on the assigned unit.
• Work with the DON to ensure compliance with state and federal regulations.
• Notify attending physicians and family members of significant changes in a resident's condition and takes follow up action as necessary.
• Monitor in-house transfers, transfers to other facilities, and assist with discharge planning.
• Maintain current and accurate documentation, assist with and maintain paperwork.
• Display a courteous attitude and respect for all residents, families, and staff.

Qualifications:

• Currently licensed as a Registered Nurse in the state of Minnesota; license is unrestricted.
• Five plus years prior RN experience preferred with at least one year in geriatrics.
• Three to five years prior supervisory experience preferred. Ability to develop/lead nursing team.
• Current CPR Certification required.
• Willingness to work with other disciplines in coordinating and monitoring resident services and activities directed toward achieving the highest practicable level of physical, mental, psychosocial, and spiritual health of our residents.
• Ability to work autonomously or collaboratively as situation dictates.
• Excellent verbal and written (read, write) communication skills.
