Occupational Therapist Assistant (COTA)

Location: GA Wellness - Elk River, MN

Salary Range: $26.50 - $33.25/Hour (Based on Experience)

Position Summary

Provides Occupational Therapy services by supporting the evaluation and treatment of residents/clients, helping develop and carry out therapy plans, and completing related documentation and communication. Works under the supervision of a licensed Occupational Therapist. Follows established occupational therapy standards, along with all facility policies, procedures, and state regulations. Services are provided in accordance with the mission of Guardian Angels.

Job Duties
  • Under the direction of the registered and/or licensed occupational therapist, optimize the physical abilities and skills of patients by assisting in the performance of screening, treatment implementation, documentation, and communication with residents, families, staff, and other professionals.
  • Assist in selecting Occupational Therapy techniques/media, adapting as needed, and consulting with the Occupational Therapist before major plan modifications.
  • Assist in establishing and modifying individual resident/client schedules with the Occupational Therapist and other team members.
  • Assist the Occupational Therapist in identifying problem lists, long‑term and short‑term goals, and treatment plans.
  • Monitor patient response to intervention and consult with the Occupational Therapist if modifications are indicated.
  • Adhere to department and resident/client schedules and adjust as appropriate for treatment regimes.
  • Adapt treatment under the supervision of an Occupational Therapist to meet individual needs.
  • Provide, recommend, or fabricate adaptive devices, orthotics, or prosthetics, and train residents, families, and appropriate staff in their use.
  • Participate in resident/client, family, and staff education.
  • Coordinate the total rehabilitation program with other team members.
  • Maintain documentation of occupational therapy services in accordance with facility policies.
  • Attend in‑service training and facility meetings as required.
  • Provide in‑service education to the resident/client care team and the community.
  • Assume responsibility for continuing education and professional development.
  • Submit billing, payroll, and expense documentation accurately and timely.
  • Assist in collecting statistical data as directed.
  • Provide treatment in accordance with established standards, department procedures, and efficiency standards.
  • Consistently demonstrate sound judgment in evaluation, planning, implementation, and follow‑up of therapy programs.
  • Maintain positive relationships and rapport with coworkers, residents, families, and facility personnel.
  • Participate in Continuous Quality Improvement programs as required.
  • Project a positive and professional image at all times.
  • Maintain a safe and clean work area and adhere to safety standards.
  • Comply with all Human Resources policies and organizational policies.
  • Assist in providing a dignified, respectful, resident‑centered environment.
  • Follow universal precautions, infection control guidelines, sanitation, and safety standards.
  • Perform all duties in accordance with current standards, guidelines, state and federal regulations.
  • Uphold confidentiality as required by HIPAA and Guardian Angels policies.
  • Uphold the mission, vision, and values of Guardian Angels.
  • Perform all other duties as assigned.
