Job Duties and Responsibilities:
- Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution.
- Review data entry in medical record system to ensure accuracy for billing and compliance.
- Conduct patient admission assessments, care planning, goal setting, evaluation, coordination of care, emergency/safety plans as needed per manager direction.
- Ensure the development and implementation of the individualized plan of care.
- Assist with HHCAHPS survey analysis and implementation of quality improvement activities.
- Understand and comply with policy and procedures set forth by AHC.
- Monitors operations to ensure compliance with county, state and federal regulations affecting home health care.
- Additional tasks, as assigned by manager.
- Must be licensed as a Registered Nurse (RN) in Minnesota.
Additional Requirements:
- Working knowledge of Microsoft Office suite.
- Excellent oral and written communication skills.
- Ability to read, write and speak fluently in English.
- Education and experience in the scope of services offered by the agency.
- Two years of home care experience including OASIS data collection, preferred.
- Maintain confidentiality in accordance with applicable laws and rules.
- Provide four week written notice prior to termination of employment.
