Salary Range: $70,000 - $90,000 (Based on Experience)

Provide accurate and complete assessment and documentation of resident consistent with parameters and definitions established for current MDS format. Provides assistance with resident admissions on Transitional Care Unit. Services are provided in accordance with the mission of Guardian Angels.

• Assess/document resident condition utilizing MDS and supplementary documentation tools.
• Provide recommendations regarding appropriateness or lack thereof of projected Assessment Reference Date.
• Assist with admission process for residents.
• Participates in the Resource Nurse Rotation.
• To assist in providing a dignified, respectful, resident-centered environment for residents.
• Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities.
• All duties will be performed in accordance with current standards, guidelines, state and federal regulations.
• Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPPA.
• Uphold the mission, vision, and values of Guardian Angels of Elk River.
• Performs all other duties as assigned.

• Current Minnesota Registered Nurse licensure.
• Minimum of one year experience in geriatric nursing.
• Prior experience in Experience in ICD 10 coding and MDS scheduling
• Prior experience with completing Minimum Data Set (MDS); familiarity with prospective payment system.
• CPR trained and First Aid training or willingness to obtain within 6 months of employment.
• Ability to complete assignments timely.
• Previous computer experience required.
• Excellent verbal and written (read, write) communication skills.
• Self-motivated, ability to prioritize.
• Maintains good attendance and punctuality.
