What You'll Be Doing- Provide skilled RN home health visits throughout the Twin Cities service area
- Perform wound care
, medication management, injections, and other skilled nursing services - Educate patients and caregivers on disease management and recovery
- Collaborate with physicians, therapists, and interdisciplinary care teams
- Support multiple territories based on patient needs and census
- Help ensure patients receive timely, uninterrupted care
- Active Minnesota RN license
- Experience in home health
, hospital, rehabilitation, or skilled nursing preferred - Strong assessment and critical thinking skills
- Comfortable working independently in a field-based environment
- Flexible and adaptable to changing daily assignments
- Reliable transportation and willingness to travel throughout the Twin Cities Metro
