Independent Living Skills Coach

Join Sevita as an Independent Living Skills Coach in our Community & Social Services team. In this role, you'll support individuals with disabilities or behavioral health needs to build daily living, social, and community integration skills. You'll coach people in areas such as budgeting, personal care routines, transportation, and household management, using person-centered plans. Collaborate with clinical and support teams, document progress, and advocate for client independence.

Responsibilities

• Provide one-on-one and small-group coaching in daily living and social skills
• Support individuals with community integration, transportation, and appointment coordination
• Implement person-centered service plans and track progress toward goals
• Document services, outcomes, and incidents accurately and on time
• Collaborate with clinical, family, and community partners to support individuals
• Teach budgeting, household management, and self-advocacy skills
• Assist with behavior support strategies as directed by care teams
• Ensure client health, safety, and dignity in all settings
• Participate in ongoing training and professional development
• Advocate for individuals' rights, preferences, and independence
