Job Summary

Responsibilities- Coordinate and deliver primary care in collaboration with the Practice Site Physician, working within approved scope of practice.
- Obtain complete medical history and physical data on patients.
- Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed.
- Perform a complete physical exam, record findings, and collaborate with physicians in managing the medical needs of patients.
- Order appropriate laboratory and diagnostic procedures.
- Educate and advise patients regarding health and illness prevention, and recommend community resources to meet patient and family needs.
- Support patient access and care coordination through triage, follow-up, and management of urgent situations.
- Provide clinical and administrative assistance to BMG programs, e.g., education, follow-up, and related activities.
- Participate in health education, maintaining related records, and preparing statistical reports as needed.
- Participate in interdisciplinary care conferences and assist with referrals and dissemination of information to appropriate vendors.
- Assist in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.
- Attend and participate in department meetings and be accountable for all information shared.
- Complete mandatory education, annual competencies and department‑specific education within established time frames.
- Complete annual employee health requirements within established time frames.
- Maintain license/certification, registration in good standing throughout the fiscal year.
- Maintain current BCLS (CPR) and other certifications as required by position/department.
- Consistently utilize appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
- Adhere to regulatory agency requirements, survey process and compliance.
- Comply with established organization and department policies.
- Be available to work overtime and additional shifts or schedules when required.
- Graduate of an accredited Physician Assistant or Nurse Practitioner master’s program.
- Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date.
- BLS obtained within one month of hire date or job transfer date if required (American Heart Association or American Red Cross accepted).
- Current BLS and ACLS certifications if required.
- Ability to work collaboratively in a fast‑paced, team‑oriented environment.
- Thorough knowledge of practice standards, policies, and procedures of BMG and the appropriate professional organization.
- High level of analytical skills necessary to assess the patient and participate in follow‑up care planning.
- Interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals and paraprofessionals.
- High level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
- Work in a clinical environment requiring physical exertion, frequent changes in job demands, and exposure to biohazards.
- Physical ability and stamina to walk/stand for prolonged periods, push carts/wheelchairs, lift up to 50 pounds, position/lift patients up to 35 pounds without assistance, provide CPR, etc.
