Job Summary
We're looking for an RN Care Coordinator to join our team and make a meaningful impact by helping patients navigate their healthcare journey from hospital admission through recovery.
Experience- High-Risk Patient Management
- Care Coordination and Transitions of Care
In this role, you'll collaborate with physicians, hospitalists, emergency department teams, social workers, and outpatient providers to ensure seamless, patient-centered care. You'll help coordinate discharges, medication reconciliation, follow-up appointments, and communication across the entire care continuum.
Qualifications- Minimum of 3 years of clinical experience
- Strong critical thinking, communication, and relationship-building skills
- Passion for improving patient outcomes and reducing unnecessary hospital utilization
