MDS Coordinator

Trilogy Health Services seeks an MDS Coordinator to lead resident assessment and care planning processes in our senior living and long-term care community. In this role, you will manage MDS assessments, ensure regulatory compliance, and support accurate documentation tied to reimbursement. You will collaborate closely with nurses, therapists, and physicians to develop individualized care plans that reflect resident needs and preferences.

Responsibilities

• Coordinate and manage the MDS assessment process for residents to ensure accuracy and regulatory compliance
• Collaborate with nursing and interdisciplinary teams to develop and update individualized care plans
• Review clinical documentation and enter MDS data accurately and on time
• Monitor quality indicators and support initiatives to improve resident outcomes
• Educate staff on MDS guidelines, documentation standards, and care planning requirements
• Communicate with residents, families, and care teams to support care goals and regulatory needs
• Assist with audits, surveys, and reporting related to MDS and reimbursement
• Maintain current knowledge of federal and state long-term care regulations
