Beacon Health System is hiring a full-time days Physician Aligned – Oncology Nurse Navigator for our Beacon Medical Group located in Elkhart, IN. We're proud to offer a sign-on bonus of $5,000 one-time (not base pay).

As an Physician Aligned – Oncology Nurse Navigator, you will report to the Manager or designee. Manages resources through interdisciplinary collaboration to achieve optimal patient outcomes for oncology patients. Coordinates research data, community physicians and Beacon Health System resources into a seamless model of access and care that benefits patients, participating physicians and family members. Serves as a hospital and Beacon Medical Group ambassador from the patient's first contact to follow-up post-treatment for coordination of care.

Provides education regarding services available to oncology patients through and among the different specialty departments of the Cancer Center. Develops survivorship care plans for all eligible oncology patients and provides teaching and resources for optimal survivorship care.

Oncology Nurse Navigator – Physician Aligned,

Job Responsibilities:

  • Coordinate oncology patient care across the full continuum
  • Guide adult oncology patients through the healthcare system
  • Assess newly diagnosed patients/families for needs and barriers to care
  • Collaborate with physicians and care teams to implement care plans and achieve outcomes
  • Serve as liaison between patients, families, and interdisciplinary teams
  • Educate patients and families on diagnosis, treatment plans, and care processes
  • Identify and address barriers (insurance, transportation, childcare, financial, language, etc.)
  • Initiate and coordinate referrals to internal departments and community resources
  • Partner with community agencies to support patient care needs
  • Participate in interdisciplinary care conferences to plan interventions
  • Promote patient compliance through education and support of treatment plans
  • Monitor and report variance trends to leadership and physician reviewers
  • Track, gather, and report patient outcomes, including post-procedure follow-up
  • Develop and review survivorship care plans, including long-term follow-up guidance
  • Conduct community health screenings and educational outreach
  • Contribute to Oncology Care Committee and required reporting (e.g., ACS)

Quality & Reporting:

  • Compile, analyze, and report utilization and performance data
  • Prepare summaries and reports for Oncology Care Committee review

Clinic Coordination:

  • Manage patient flow within oncology and multidisciplinary clinics
  • Provide direct nursing care in multidisciplinary settings
  • Coordinate scheduling of procedures and follow-ups

Survivorship Program:

  • Identify eligible patients via Cancer Registry
  • Develop, deliver, and review survivorship care plans with patients/families

Additional Responsibilities:

  • Complete special projects and other assigned duties to support department effectiveness

What You Bring:

As Physician – Aligned Oncology Nurse Navigator, this position requires thorough knowledge of oncology clinical care practices, procedures, and techniques, as well as strong understanding of community agencies, services, programs, and resources available to support patients and families. Possesses knowledge of counseling techniques and the ability to identify and connect patients with appropriate resources. Demonstrates strong verbal, written, and listening communication skills to effectively counsel patients and families, along with excellent interpersonal skills to maintain cooperative, respectful, and professional relationships with patients, families, physicians, staff, and the public.
