This position is responsible for providing information and assistance, options counseling, enrollment/disenrollment counseling, long‑term care program eligibility determination and advocacy to the general public, specifically older adults, adults with disabilities and youth age 17 years and 6 months, within the scope of the Aging & Disability Resource Center.

Essential Duties And Responsibilities
  • Facilitate consumer access to service, collaborate with contracted providers, monitor purchased services for consumers, and determine ongoing need and effectiveness of services based on assessment findings.
  • Evaluate and assess individuals and families with psychosocial and/or physical difficulties; develop and carry out service plans; coordinate and monitor services and treatment; review and terminate services.
  • Administer mandated functional and financial screens within state and federal guidelines to determine program eligibility.
  • Provide short‑term case‑management services for assigned caseload, including face‑to‑face contact, in‑home or community visits.
  • Complete annual protective placement reviews for persons who are elderly, physically disabled or intellectually disabled and are enrolled in a long‑term care program.
  • Complete access and abuse‑neglect reports according to State statute and forward to APS team for screening review.
  • Advocate for consumers and provide community education; educate consumers and their support system to help achieve personal goals and maximum potential.
  • Incorporate current best‑practice models into daily service provisions; work with individuals, families, and significant others when indicated.
  • Consult, coach and collaborate with team members, families, individuals, providers, and other Health and Human Services staff.
  • Collaborate with other programs and individuals across the department to ensure coordination of care.
  • Participate in trainings to update knowledge and skills, maintain certifications and personal licensure.
  • Ensure compliance with program standards and maintain necessary documentation as required by the County Department of Health & Human Services, the Department of Health Services, and state and federal statutes.
  • Participate in Long Term Care division functions related to the general operation, including trainings, supervision, progress meetings for consumers, division and program staff meetings, supervision, resource development, marketing, and outreach to the general public.
  • Serve on related work groups, projects, committees, and/or community projects as assigned.
  • Demonstrate a commitment to county safety and risk‑management efforts.
Qualifications
  • Education:

    Bachelor's degree in social work or a related field and at least one year of relevant prior experience required;
    Master’s degree preferred.
  • Experience:

    Working with older adults and disabled populations preferred; knowledge of long‑term care programs (Family Care, IRIS, AFCSP, SSIE, MA) preferred.
  • Knowledge:
    Trauma‑Informed Care Principles essential.
  • Certificates, Licenses and Registrations:
    Valid Driver’s License; minimum liability auto insurance coverage; reliable personal vehicle for business travel;
    Wisconsin AIRS Certification for I & R Specialist in Aging/Disability (CIRS‑A/D) required within one year of hire;
    Wisconsin DHS Adult Long Term Care Functional Screener certification preferred at hire or within two months.
