Position Summary
The Admissions Coordinator acts as the primary liaison between hospitals, families, and our interdisciplinary clinical team. This role is responsible for managing the entire admissions process, from initial inquiry to final admission, ensuring all regulatory requirements are met while maintaining a high-occupancy census. The ideal candidate is compassionate, detail-oriented, and able to work in a fast-paced, high-stress environment.

Responsibilities- Serve as LHCC’s primary representative to the community by handling tours, telephone inquiries, and all potential admission contacts.
- Manage all incoming referrals from hospitals, discharge planners, case managers, and community partners.
- Review clinical, medical, and psychosocial information to determine whether the facility can meet a prospective resident’s needs.
- Guide residents and families through the entire admissions process, including explaining state and federal requirements, facility policies, financial responsibilities, available services, and assisting with all paperwork.
- Ensure all admission forms are completed accurately and on time, including insurance and payor information, and document all admission activities electronically.
- Maintain compliance with Preadmission Screening and Resident Review (PASRR) requirements.
- Collaborate with all LHCC departments to ensure a smooth, patient-centered, and trauma-informed admission experience.
- Support marketing and census development efforts to maintain target occupancy.
- Maintain strong knowledge of all payer types and provide education regarding potential insurance coverage.
- Coordinate Medicare, Medicaid, and private insurance verification for all admissions.
- Maintain a backup process for after-hours referrals and admissions, and verify accuracy of forms completed by backup staff.
- Build and sustain positive relationships with physicians, hospital discharge planners, and other key referral sources.
- Participate on the Quality Assurance Committee and report on admission and discharge trends.
- Stay current on trends, regulations, and best practices in long-term care through ongoing education and professional development.
- Provide facility tours and participate in off-site marketing events.
- Engage in community activities that strengthen and support LHCC’s presence and partnerships.
- Support county safety and risk management initiatives.
- Bachelor’s degree in Business, Marketing, Social Work or related field or an Associate Degree in Nursing and one to two years of experience or an equivalent combination of education and experience with nursing home admissions and marketing in a long term care setting.
- Minimum 1-2 years of experience in healthcare admissions, specifically in a skilled nursing or long-term care setting.
- Strong understanding of Medicare, Medicaid, and managed care payor sources.
- Possess an understanding and/or working knowledge of medical and nursing care management, medical terminology, rehabilitation for long term care residents, and medical record organization and management.
- Knowledge of policies or procedures pertaining to resident care practices, organizational structures and departmental functions.
- Wisconsin Social Worker license (CSW) or Wisconsin Nursing license (RN/LPN).
