Licenses Pracical Nurse - LPN

Position – Licensed Practical Nurse (LPN)

Location: Holton Manor – Elkhorn, WI (On-site)

Status: Full-Time | Part-Time | PRN

Schedule: Days | PMs | NOCs | Weekends – True Flexible scheduling-you choose what fits your schedule

The LPN must be clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.

PLUS $2.00 per hour PM and $2.50 per hour NOC shift differentials

Provide safe, effective nursing care per care plans and physician orders

Administer medications and treatments accurately

Assess residents and report changes promptly

Document care in the EMR

Monitors performance and mentor CNAs

Ensure infection control and safety compliance

Completes all other duties as assigned.

Required:

Active LPN license in state of Wisconsin or licensed in any U.S. state and be eligible for Wisconsin licensure by reciprocity. (in good standing)

CPR certified

Ability to meet physical and job-related requirements

Must be English literate and possess excellent written and verbal communication skills

Preferred:

Long-term care or skilled nursing experience

Clinical experience

Hiring manager contact within 1–2 business days

1–2 interviews with leadership and mentor

Electronic onboarding and paid orientation
