Position: RN Care Manager - Rock County

RN Care/Case Manager – Full Time

Community Care is offering a $8,000 sign‑on bonus for full‑time RN Care/Case Manager to join the day shift. This is a full‑time opportunity for Rock County, based out of our Walworth office. Hybrid work is available.

Responsibilities
  • Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem‑solving related to member appointments, transportation, and home care needs.
  • Conduct phone and in‑person assessments using decision tools and resources to identify immediate and potential health needs.
  • Complete all required documentation and charting and assist in the development and maintenance of member records.
Requirements
  • Minimum 2 years of personal, clinical leadership, and/or employment experience providing care or service for a frail or elderly population required; 2 years of recent case management preferred.
  • A current Wisconsin Registered Nurse license in good standing.
Schedule
  • 8:00 a.m. – 4:30 p.m., 40 hours a week.
