RN Care/Case Manager – Full‑time Day Shift. Hybrid work available.

• Collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members, including collaborative problem solving related to member appointments, transportation, and home care needs.
• Conduct phone and in‑person assessments using decision tools and resources to identify immediate and potential health needs.
• Complete all required documentation and charting and assist in the development and maintenance of member records.

• Minimum 2 years of personal, clinical leadership, or employment experience providing care or service for a frail or elderly population. Two years of recent case management preferred.
• A current Wisconsin Registered Nurse license in good standing.

8:00 a.m. – 4:30 p.m., 40 hours a week.
