Location:Holton Manor –Elkhorn, WI (on-site position)

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members.

• Weekend Warrior Program – choose one of the following 3 levels:
  • base hourly wage +$5.00/hour (every other weekend)
  • Pro Level: base hourly wage +$7.00/hour (3 weekends per month)
  • PLUSshift differentials - $2.00 per hour PM shift and $2.50 per hour NOC shift

• Provide safe, effective nursing care per care plans and physician orders
• Administer medications and treatments accurately
• Assess residents and report changes promptly
• Document care in the EMR
• Monitors performance and mentor CNAs
• Ensure infection control and safety compliance
• Completes all other duties as assigned.

Required:

• ActiveLPN license instate of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity.(in good standing)
• CPR certified
• Ability to meet physical and job-related requirements
• Must be English literate and possess excellent writtenandverbal communication skills

Preferred:

• Long-term care or skilled nursing experience
• Clinical experience
