• Provide nursing care within the licensed practical nurse scope, including routine meds administration, vital signs, and assessment
• Monitor residents for changes in condition and report promptly to the charge nurse, documenting findings accurately
• Perform wound care, ADLs assistance, and standard nursing procedures as delegated
• Maintain and update resident records and documentation in the Point-Click-Care electronic charting system
• Collaborate with the interdisciplinary team to support care plans and ensure resident safety and comfort
• Adhere to infection control, safety protocols, and regulatory requirements; respond to emergencies in a timely manner

• Active Licensed Practical Nurse license in West Virginia
• Point-Click-Care experience is required
• Current clinical nursing experience as an LPN
• Ability to work the 3x12 night shift and adapt to a two-week schedule rotation (2 days one week, 4 days the next)
• Travel assignment within a 100-mile radius of the assignment location
• Ability to work in a long-term care setting and follow care plans for residents
