• Provide direct nursing care to residents in a long-term care setting under the supervision of a registered nurse
• Administer prescribed medications and treatments according to policy and clinician orders
• Monitor vital signs, assess health changes, and promptly report to the supervising RN
• Assist residents with activities of daily living, promoting safety, dignity, and independence
• Document care provided accurately in the chart and ensure proper handoffs at shift change
• Collaborate with the interdisciplinary team and participate in care planning and routine quality tasks

• Active LPN license in West Virginia
• Ability to work 12-hour shifts in a long-term care environment
• Strong communication and teamwork skills
• Eligibility to complete required credentialing and orientation processes
• Willingness to participate in the assigned weekend rotation and fixed shift schedule
