We are seeking a compassionate, organized, and motivated Foster Care Coordinator to support children in foster care while partnering with foster families, biological families, community providers, and the West Virginia Department of Human Services. - Manage a specialized foster care caseload while ensuring child safety, permanency, and well‑being.
- Build meaningful relationships with foster families through ongoing coaching, guidance, and support.
- Conduct monthly home visits and maintain regular contact with children and caregivers.
- Develop, implement, and monitor individualized treatment and permanency plans.
- Coordinate services with schools, healthcare providers, therapists, courts, and community agencies.
- Facilitate multidisciplinary team meetings and family visitation.
- Prepare timely documentation, progress reports, court reports, and electronic case records.
- Complete annual foster home reevaluations and ongoing foster family support activities.
- Participate in court proceedings and provide professional testimony when required.
- Respond to crisis situations and participate in an after‑hours on‑call rotation.
- Support accreditation, Continuous Quality Improvement (CQI), and agency strategic initiatives.
- Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a closely related field from an accredited college or university.
- Current West Virginia Social Work license.
- Valid driver’s license with an acceptable driving record.
- Ability to travel throughout assigned service areas.
- Successful completion of all required background checks.
- Experience in foster care, child welfare, behavioral health, or family services.
- Knowledge of trauma‑informed care, child development, and permanency planning.
- Strong assessment, documentation, organizational, and communication skills.
- Experience working collaboratively with multidisciplinary teams and community partners.
