Represents the VA with professionalism and respect to both internal and external stakeholders.

Acts as a subject matter expert on VA and/or community resources.

Collaborates with other service providers in assessing the Veteran's needs as appropriate.

Works closely and coordinates with other PACT Social Workers in the health care system, along with providing consultation and education to VA staff and community-based service providers, including medical residents and social work students.

Educates Veterans and/or caregivers about the available services and assists them in establishing the appropriate referrals based on the Veteran's preference or that of their authorized surrogate decision-maker.

Conducts outreach and engagement activities with Veterans experiencing homelessness, housing instability, behavioral health concerns, or difficulty engaging in healthcare services.

Establishes and maintains collaborative relationships with community shelters, transitional housing programs, law enforcement agencies, treatment courts, food pantries, outreach teams, and other community partners serving vulnerable Veteran populations.

Provides consultation services regarding the psychosocial needs of homeless and justice-involved Veterans to PACT staff and community providers.

Utilizes knowledge of community resources, makes referrals to the community and other government agencies as appropriate, and coordinates services.

Utilizes current VA and community social service programs and enhances the content to the benefit of Veterans.

Serves as a liaison with community agencies, assists Veterans (or caregivers) with referrals for services from community and government agencies, and helps coordinate these services, when appropriate.

Coordinates with other VA staff and community agencies to assist in problem-solving as needed.

Runs psychotherapeutic and psycho educational groups for Veterans, caregivers, and/or family members.

Conducts direct outreach, assessment, case management, and care coordination for Veterans experiencing homelessness or housing instability and refers Veterans to appropriate VA and non-VA services.

Assists eligible Veterans with enrollment into VA healthcare and linkage to housing, mental health, substance use disorder treatment, financial assistance, transportation, and supportive community services.

Coordinates with VA Homeless Programs, including HUD-VASH, HCHV, Grant and Per Diem (GPD), Domiciliary Care for Homeless Veterans (DCHV), VJO, and community providers to facilitate continuity of care and housing stability. As appropriate, this may entail coordinating with other VA facilities due to the clinic's location.

Collaborates with community agencies, shelters, outreach teams, law enforcement, and justice partners to improve access to care and reduce barriers for Veterans experiencing homelessness.

Participates in community outreach events, stand downs, and field-based engagement efforts to identify and connect vulnerable Veterans with VA healthcare and supportive services.

Conducts psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.

Functions as an independent provider within the PACT.

Pay:
Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Retirement:
Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
