Social Worker II - MSW - Behavioral Health; M-F - 8a-4:30p
PRIMARY FUNCTION:
The Social Worker provides professional social work services to patients and families across Behavioral Health, Maternal–Infant, and medical settings. Responsibilities include psychosocial assessment, crisis intervention, discharge planning, counseling, and coordination of community resources. The Social Worker collaborates with interdisciplinary teams to promote safe transitions of care, optimize patient outcomes, and address social determinants of health.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Behavioral Health Social Work
- Conduct comprehensive psychosocial assessments for patients with mental health or substance-use conditions.
- Provide crisis intervention, safety planning, de-escalation, and behavioral support.
- Facilitate individual, family, and group therapeutic interventions.
- Coordinate inpatient and outpatient psychiatric placements and community resources.
- Support involuntary commitment processes in accordance with Maryland regulations.
- Document assessments, interventions, and treatment plans in the EMR.
- Participate in interdisciplinary rounds and treatment planning.
Maternal–Infant / Perinatal Social Work
- Complete psychosocial assessments for pregnant, postpartum, and neonatal patients.
- Address high‑risk factors including substance use, intimate partner violence, adoption planning, homelessness, and teen pregnancy.
- Provide education and counseling regarding perinatal mood disorders, bonding, and attachment.
- Collaborate with OB/GYN, Pediatrics, and community agencies (WIC, home visit, Early Intervention).
- File and coordinate required reports with Child Protective Services.
- Provide support in cases of fetal or infant loss. Ensure safe discharge plans for newborns including transport, caregiver readiness, and environmental safety.
Inpatient Medical Acute Care Social Work
- Perform psychosocial assessments addressing social determinants of health.
- Assist case management with discharge planning for SNF, IRF, LTACH, home health, or community‑based support programs.
- Provide illness-adjustment counseling, healthcare navigation guidance, and family support.
- Coordinate referrals to APS, hospice, palliative care, guardianship services, and other community programs.
- Collaborate with interdisciplinary teams to reduce barriers to timely discharge.
- Maintain regulatory compliance with CMS and Joint Commission requirements.
Emergency Services
- Perform psychosocial assessments addressing social determinants of health.
- Coordinate referrals to APS, hospice, palliative care, guardianship services, and other community programs.
- Collaborate with interdisciplinary teams to reduce barriers to timely discharge.
- Address high‑risk factors including substance use, intimate partner violence, elder and child abuse, and homelessness.
- Assist case management with discharge planning for SNF, IRF, home health, or community‑based support programs.
ADDITIONAL DUTIES
- Supports the departmental educational and staff development initiatives including supervision of undergraduate/ graduate students enrolled in an accredited school of social work. As a field work supervisor, is responsible to Field Instructor/Liaison from the school of social work for evaluating student’s performance and teaches the basics of medical social work and available community resources.
- Attends regularly scheduled departmental in-service presentations regarding new resources and clinical techniques.
- Attends regularly scheduled staff meetings, i.e., team, campus, or system wide.
- Represents Christiana Care, when requested, through participation on community/state boards, committees, panels, court hearings, etc.
- Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

EDUCATION AND EXPERIENCE REQUIREMENTS:
Master's degree in social work (MSW) from a CSWE‑accredited program, required.
