What You'll Do
* Conduct medical assessments and support intake/placement decisions
* Develop and oversee individualized care plans in collaboration with physicians and interdisciplinary teams
* Monitor and update medical records, physician orders, and medication administration records
* Administer medications and ensure compliance with established protocols
* Coordinate care with primary care providers and specialists; attend appointments as needed
* Support individuals with complex or high-risk medical conditions
* Provide hands-on training to staff, caregivers, and families on medical care and equipment
* Promote overall health, wellness, and preventative care strategies
* Participate in care team meetings (IHP/POC) and contribute to ongoing health planning
* Provide guidance and support to staff regarding home care and medical equipment
* Participate in an on-call rotation as needed
What You'll Bring
Qualifications:
* Graduate of an accredited RN program
* Active RN license in the state of practice
* At least 1 year of experience working with medically complex individuals
Certifications:
* Current CPR/First Aid certification
* Valid driver's license, auto insurance, and reliable transportation (if transporting individuals)

Additional Requirements
* Ability to perform physical tasks, including lifting (up to 100 lbs occasionally)
