What You'll Do

• Conduct medical assessments and support intake/placement decisions
• Develop and oversee individualized care plans in collaboration with physicians and interdisciplinary teams
• Monitor and update medical records, physician orders, and medication administration records
• Administer medications and ensure compliance with established protocols
• Coordinate care with primary care providers and specialists; attend appointments as needed
• Support individuals with complex or high-risk medical conditions
• Provide hands-on training to staff, caregivers, and families on medical care and equipment
• Promote overall health, wellness, and preventative care strategies
• Participate in care team meetings (IHP/POC) and contribute to ongoing health planning
• Provide guidance and support to staff regarding home care and medical equipment
• Participate in an on-call rotation as needed

What You'll Bring

• Qualifications:
• Graduate of an accredited RN program
• Active RN license in the state of practice
• At least 1 year of experience working with medically complex individuals
• Certifications:
• Current CPR/First Aid certification
• Valid driver's license, auto insurance, and reliable transportation (if transporting individuals)

Additional Requirements

• Ability to perform physical tasks, including lifting (up to 100 lbs occasionally)
• Potential exposure to blood borne pathogens
• Travel as needed
