Case Manager (HIV)
Are you passionate about making a difference in the lives of individuals affected by HIV? We are committed to providing compassionate, client-centered care that empowers individuals and strengthens communities. We are currently seeking dedicated Case Managers to join our dynamic team.
Provide comprehensive case management services to individuals living with HIV. Coordinate medical, mental health, housing, and social services. Advocate for clients and support them in achieving health and wellness goals. Collaborate with a multidisciplinary team to ensure holistic care.
Must have or be eligible for a provisional MHRT/C certificate and be able to obtain a full MHRT/C within two years from time of hire. This position requires a minimum of one year of relevant work or internship experience with adults, children, and families.
