* Are you a compassionate and experienced Nurse Practitioner looking for flexible opportunities? We are seeking skilled Nurse Practitioners to partner with in performing in-home health assessments across various locations in Maine. This per diem role offers independence, competitive pay per assessment, and mileage reimbursement for travel.

* In this role, you will:
* * Perform thorough health assessments in patients' homes, including reviewing medical histories, conducting physical exams, and screening for chronic conditions.

* Educate patients on medication management, disease prevention, and overall health maintenance.

* Document relevant diagnoses and communicate findings to ensure effective care management.

* Collaborate with primary care providers to address any gaps in patient care.

* Enjoy a flexible schedule—no on-call, weekends, or holiday shifts required.

* Position Requirements:
* * Availability to complete at least 30 in-home assessments per month.

* Active Nurse Practitioner license in Maine.

* Board Certification from AANP or ANCC in Family, Adult-Gerontology, or related specialties.

* Minimum of 1 year of clinical experience, preferably with geriatric patients.

* Strong communication and interpersonal skills, with a dedication to delivering patient-centered care.

* Reliable transportation for local travel.

* Comfort with working in varied home environments.
