Job Details
• Home care and hospice setting serving patients across the Ellsworth and Bangor, ME territory,
• Caseload averages 25-30 patients per case manager; office census averages 150 patients,
• Common patient types include post-orthopedic surgery, wound care, chronic/acute CHF and COPD, post-cardiac surgery, genitourinary conditions, IV antibiotic teaching, central line care, med pre-fills, and lab draws,
• Schedule is Monday through Friday, 8 AM-5 PM, with consideration for 4x10-hour days dependent on experience,
• Weekend rotation required, typically once per month but may be more frequent depending on staffing,
• Overnight on-call rotation from 5 PM to 8 AM, typically once every 3-4 weeks,
• EMR:
Netsmart (experience preferred),
• IV pumps provided through vendor,
• May be asked to support other agency locations if Ellsworth territory census is low,
• 26-week contract,
Job Requirements
• Active ME RN license or compact RN license currently in hand,
• Current BLS certification,
• Valid driver's license required; must have access to own vehicle for patient-to-patient home visits,
• Minimum 1 year of hospice or home health experience required,
• Clinical skills required to manage hospice diagnoses including wound care, CHF, COPD, central line care, IV antibiotic teaching, and lab draws,
• Flu vaccination required for eligibility to start,
• Must provide 2 professional references from within the past year; at least one reference must be from a supervisor, manager, or director (charge nurse acceptable; peer references not accepted),
• Full SSN required for consideration,
• Copies of all certifications required for consideration,
• Driver's license must be submitted as a separate document for consideration,
• Permanent address must be 100+ miles south/southwest of Bangor, ME,
• Must not have previously worked at any Northern Light Health facility in a permanent capacity,
Additional Information
• Provide case management and direct patient care to hospice patients in their homes across the Ellsworth/Bangor territory,
• Responsibilities include wound care, IV antibiotic teaching, central line care, medication pre-fills, and lab draws,
• Support team includes a clinical manager, scheduling team, and lead support staff,
• One-week orientation provided,
• Dress code is business casual or scrubs; no specific color requirement,
• Holiday coverage required when assignment coincides with major holidays (New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas),
• All time off requests require manager approval,
• First-time travelers considered only if they have prior home health and hospice experience,
• Travelers living 100+ miles north of Bangor, ME are considered on a case-by-case basis with individual approval
