Job Overview:

As a Case Manager, you will assist clients in finding services, programs, and resources to help them gain independence and work towards their established goals; while also providing support, advocacy, referral, and assessment services.

  • Understand each client's diagnosis and recommend services based on individual assessment.
  • Serve as a liaison between clients and all agencies providing services, programs, and community resources. Obtain all necessary releases to provide or receive information as needed.
  • Meet with other service providers to plan and coordinate treatment to ensure the integration of the treatment across the client's home, school, and community.
Requirements:
  • Must have an MHRT-C certification and bachelor's degree
  • At least 1 year of experience providing social services
  • Clean background check
  • Must be organized, detail-oriented, and able to work independently in the field
