Clinical Pharmacist Specialist - Location:
Elmhurst Hospital - Full Time - 40 hours/week

Education, Experience and/or
Skills Required:
- Clinical residency with a minimum of 2 years of clinical pharmacy practice experience required, (or)
- Minimum five years of clinical pharmacy practice experience
- Pharm
D. from an ACPE accredited School of Pharmacy
Required:
- Current Illinois Registered Pharmacist License or Illinois Pharmacist Licensure in progress and completed within 90 days of hire.
- Current CPR Certification issued by either American Heart Association within six weeks of hire into position.
- Current Advanced Cardiac Life Support certification or within 90 days of hire into position.
- Demonstrates the knowledge and skills necessary to provide care appropriate to the patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
- Ensures safe, appropriate, cost effective drug therapies for patients according to established policies, procedures and protocols.
- Provides education and information on pharmaceutical care and drug therapy.
- Contributes to the quality and effective operation of the department.
- Maintains competency required for current job title/position.
- Perform staff pharmacist duties when needed.
