- Deliver high-quality, patient-centered care for a home dialysis program
- Educate, train, and support patients and caregivers performing dialysis at home
- Train patients and caregivers on home hemodialysis procedures, including equipment setup, treatment initiation, monitoring, and troubleshooting
- Provide education on infection control, vascular access care, and emergency protocols
- Conduct initial patient assessments, including medical history reviews and home environment evaluations
- Develop and implement individualized care plans with the interdisciplinary team
- Provide routine training, education, and guidance to patients and caregivers
- Monitor patient progress through follow-ups and assessments, adjusting care plans as needed
- Maintain accurate and timely documentation in compliance with state and federal regulations, including CMS guidelines
- Ensure patient care aligns with organizational policies and quality standards
- Train patients on dialysis equipment maintenance and use
- Educate, train, and help coordinate delivery and inventory management of supplies
- Serve as a resource and advocate for patients and caregivers
- Facilitate communication among patients, physicians, and healthcare providers
- Provide ongoing education for managing hypotension, bleeding, and equipment malfunction
- Participate in Quality Assurance & Performance Improvement or CQI as directed
- Record arrival and departure time via ADP's time and attendance system
- Promote and follow the company's mission, vision, and core values
- Receive and act on feedback from supervisors, peers, and clients
- Report incidents immediately to the supervisor
- Check work email and respond appropriately and promptly
- Maintain regular and predictable attendance and punctuality
- Perform other duties as assigned
- Initially report to the VP, Clinical Quality and ultimately to the lead for Home Dialysis Services
- Perform work in hospitals, training centers, patient residences, or medical units of post-acute or correctional facilities
- Work with medical and standard office equipment, with possible exposure to chemicals and blood-borne pathogens
- Talk, hear, stand, walk, handle or feel objects, reach with hands and arms, and push, pull, or lift supplies up to 50 pounds
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Minimum of 12 months current experience in hemodialysis, including home hemodialysis
- Experience in Peritoneal Dialysis preferred, but not required
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
- Must meet the state or facility health mandates, if applicable
- Strong knowledge of dialysis operating requirements
- Strong clinical assessment and critical thinking skills
- Excellent communication, teaching, and interpersonal skills
- Ability to work independently and manage multiple patients in a remote setting
- Proficiency with electronic medical records (EMRs) and Microsoft Office Suite
- Ability to travel to patients' homes across the assigned state
- Ability to work remotely and in an office setting when required
- Valid driver’s license and reliable transportation
- Ability to lift and move dialysis equipment and supplies, as required
- Ability to be flexible and adhere to innovative scheduling
- Ability to take on-call shifts as needed
Demonstrates expertise in delivering patient-centered care for home dialysis, including training on hemodialysis procedures and equipment management. Proficient in clinical assessment, care plan development, and compliance with healthcare regulations.
