Licensed Practical Nurse

Corning Center is seeking a compassionate Licensed Practical Nurse to provide high-quality care to residents in our skilled nursing and rehabilitation facility in Corning, NY. The LPN will deliver direct bedside care, administer medications and treatments, monitor resident conditions, and accurately document in clinical records. You will collaborate closely with RNs, physicians, therapists, and CNAs to support individualized care plans and promote resident comfort, safety, and dignity.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per provider orders and facility policies
• Monitor and document vital signs, resident conditions, and responses to treatment
• Assist with development and implementation of individualized care plans
• Collaborate with RNs, physicians, therapists, and CNAs to coordinate care
• Maintain accurate and timely electronic and written clinical documentation
• Educate residents and families on care plans, medications, and health needs
• Promote resident safety, comfort, dignity, and quality of life
• Support infection control, regulatory compliance, and facility policies
• Participate in shift reports, care conferences, and interdisciplinary meetings
