About This RoleThe Oncology Clinical Pharmacy Specialist is involved with the care of cancer patients throughout their entire treatment journey. Working closely with oncologists, fellows, nurse practitioners,s and other members of the healthcare team, this pharmacist ensures treatment plan accuracy, optimizes supportive care measures,s and serves as the definitive oncology pharmacy expert for the entire oncology service line. This role also includes a strong teaching and mentorship component, serving as a preceptor for pharmacy residents and students and as a subject matter expert for Oncology Fellows during their training.
Responsibilities- Establish a collaborative practice program with oncologists across identified areas of need,eed including supportive care measures for patients receiving chemotherapy on both an inpatient and outpatient basis.
- Conduct staff development programs and educational services for pharmacy staff, nursing staff, oncologists,s and other healthcare professionals.
- Serve as the subject matter expert in oncology, completing drug information questions for the oncology service line and reviewing Electronic Health Record care plans to ensure consistency with current national guidelines.s
- Round with Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy according to established national evidence-based guidelines, including recommendations on correct medications, monitoring outcomes, dose adjustments, and supportive care measures
- Provide patient-specific medication education using the teach-back method for new oral oncolytics and intravenous chemotherapy as needed.
- Review oncology care plans within the Electronic Health Record, including new care plans requested by oncologists and continual review of existing care plans for updates and appropriateness.
- Identify and evaluate medication-related problems based on clinical presentation, available history, or laboratory data
- Serve as a preceptor to pharmacy residents and students
- Actively participate in Oncology Service Line meetings to improve safety, efficacy,cy and quality of cancer care
- Promote rational, cost-effective drug therapy with a focus on cost avoidance and cost savings within the Oncology Service Line
- Doctor of Pharmacy, PharmD, required
- Active Delaware Pharmacist License required
- Board Certified Oncology Pharmacist, BCOP, required
- PGY1 Residency or equivalent of at least 3 years of clinical pharmacy practice OR PGY2 Residency in Oncology or equivalent of at least 3 years of oncology practice required
- Minimum 3 years of hospital experience with completion of clinical training in oncology
- Completion of a PGY2 ASHP-accredited Pharmacy Residency in a related area of expertise
- Completion of a PGY1 ASHP-accredited Pharmacy Residency plus minimum 3 years of experience in oncology and BPS Board Certification in Oncology
- Minimum 5 years of experience in oncology and BPS Board Certification in Oncology
