PRN Speech Language Pathologist Adult Elyria

• Organizes and conducts Speech‑Language Pathology services to facilitate the (re) habilitation of people with physical and/or psychosocial disabilities.
• The Speech‑Language Pathologist directs the patient’s participation in selected activities/treatments to promote and/or maintain good health and communication.
• Develops and implements the Speech‑Language Pathology plan of care to facilitate the habilitation or rehabilitation of people with speech, language, cognitive‑linguistic, psychosocial, and swallowing disabilities.
• Directs patient participation in selected activities to establish, enhance, and restore speech, language, cognitive‑linguistic, psychosocial, and swallowing abilities.
• Performs evaluations, develops an effective treatment plan appropriate to a patient’s rehabilitation needs within the care continuum, and initiates and participates in the discharge planning process.
• Educates patient, family, and caregiver in applicable health information, treatment, and prevention.
• Performs other department activities such as documentation, billing, and support of department operations.
• Meets minimum requirements as required by current hospital and department PRN policies.
• Performs other duties as assigned.
• Complies with all policies and standards.

• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for patients.
• Annual training, the UH Code of Conduct, and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
• This position is PRN.
• Will provide coverage for PTO, LOA, and staffing shortages.
• The location is Elyria Medical Center.
