Community Health Worker Wage: $18/hour starting wage. Full-time/40 hours

Elyria Campus, Medical Clinic Division Position The Community Health Worker (CHW) is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, this position's activities will promote, maintain, and improve the health of patients and their families.

The CHW provides social support and informal counseling, advocates for individuals and their community health needs, and provides services such as basic first aid, blood pressure screenings, and community referrals to appropriate wrap-around services to meet the client’s needs. This position works directly with the medical staff to ensure the client’s overall well-being is addressed. The CHW will be involved in the interdisciplinary team as a case manager for medical clients within our prenatal and medical clinic.
