Job Summary
Always Best Care of Greater Cleveland, a leading provider of senior care services, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Lorain County As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to make a meaningful difference in the lives of our clients while working in a dynamic and rewarding environment.
Responsibilities
Provide high-quality, patient-centered care to clients in their homes, as part of our skilled home health care services
Conduct patient assessments, and implement interventions as needed
Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
Perform wound care, and provide other skilled nursing interventions as ordered by physicians
Document patient care and progress accurately and thoroughly
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions

Requirements
Current LPN license in the state of Ohio
Strong communication and interpersonal skills
