Home Health Registered Nurse (RN) | Morning shift | Lorain, County

Bright Star Care is seeking a compassionate and experienced Registered Nurse (RN) to join our growing clinical team.

You'll provide skilled nursing care in the home, support new client admissions, assist with urgent staffing needs, and collaborate with an interdisciplinary team to deliver exceptional patient outcomes.

Schedule/

Location:

  • Elyria Lorain
  • 8am to 4pm:
    Monday through Friday
  • 8am to 4pm:
    Saturday and Sunday
  • Overtime opportunities available
  • Must also be available to travel throughout the West Cleveland service area
What You'll Do Skilled Nursing Care
  • Complete comprehensive patient assessments and reassessments
  • Develop and implement individualized Plans of Care
  • Provide skilled nursing services including:
    • Wound care
    • Medication administration
    • Injections
    • Infusions
    • Catheter care
    • Other nursing treatments within RN scope of practice
  • Monitor patient conditions and communicate changes promptly
  • Maintain accurate and timely clinical documentation
  • Follow all infection prevention and safety protocols
Pediatric & Home Health Care
  • Provide skilled nursing care for pediatric and adult patients
  • Infant pediatric care
  • Seizure monitoring
  • Support patients requiring assistance with Activities of Daily Living (ADLs), including:
    • Bathing
    • Dressing
    • Feeding
    • Toileting
    • Mobility and transfers
Case Management & Care Coordination
  • Coordinate care with physicians, families, therapists, and interdisciplinary teams
  • Support new client admissions and transitions of care
  • Review patient progress and adjust care plans as needed
  • Ensure compliance with Bright Star Care policies, Joint Commission standards, and state and federal regulations
Clinical Leadership
  • Conduct supervisory visits
  • Complete caregiver competency evaluations
  • Provide clinical education and support to caregivers and nursing staff
  • Assist with orientation and ongoing skills validation
  • Participate in quality assurance and chart review activities
Qualifications Required
  • Current unrestricted Ohio Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum 3 years of clinical nursing experience
  • Minimum 1 year of case management experience
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation with current auto insurance
  • Ability to travel throughout assigned service areas
  • Ability to pass a background check, drug screen, and other pre‑employment requirements
Preferred
  • Home Health experience
  • Pediatric nursing experience
  • Case management experience
Physical Requirements
  • Ability to stand, walk, bend, kneel, reach, push, and pull throughout the workday
  • Ability to safely lift up to 50 pounds with or without reasonable accommodation
