Seeking a House Calls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in‑home health assessments for Medicare and plan members. The role offers flexible scheduling within 8am-8pm, no on‑call, weekends, or holidays, promoting work‑life balance.

• Performing comprehensive assessments, including medical history, physical exams, and screenings
• Documenting diagnoses and communicating with primary care providers
• Recognizing urgent situations and providing patient education on disease management and social factors
• Referring patients to appropriate services as needed

• Active licensure and certification (ANCC, AANP, or NCCPA)
• Unrestricted driver's license and reliable transportation
• Ability to navigate home environments and perform physical assessments
• Preferred: 1+ years clinical experience, proficiency with electronic records, and strong communication skills with Medicare populations
