Alameda County:
Emeryville, California

• Provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively.
• Address areas such as home safety medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease and dementia care.
• Distribute family caregiver training.
• Serve as an educator, patient advocate, and a member of the interdisciplinary care team.

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of individual development and human behavior as they relate to illness/injury and the influence of culture on health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high‑acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

• Prior to scheduling an in‑person interview, all candidates are required to complete a computer‑literacy and competency assessment to ensure they can navigate our environment including electronic charting, shared understanding of company values, and use of our web‑based communication application.
