Responsibilities
  • Provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively.
  • Address areas such as home safety medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease and dementia care.
  • Distribute family caregiver training.
  • Serve as an educator, patient advocate, and a member of the interdisciplinary care team.

Qualifications
  • Current unrestricted CA Occupational Therapist License.
  • Current BLS/CPR Certification.
  • Valid CA Driver's License.
  • Knowledge of local community resources.
  • Knowledge of individual development and human behavior as they relate to illness/injury and the influence of culture on health care.
  • Knowledge of state and federal home‑care regulatory guidelines preferred.
  • Experience in home health care highly preferred.
  • Experience with high‑acuity patients is a plus.
  • Experience working with an interdisciplinary team is highly desired.
  • Case management fundamentals highly desired.
  • Self‑directed.

  • Outstanding interpersonal skills.
  • Leadership skills.
  • Organized.
  • Prior to scheduling an in‑person interview, all candidates are required to complete a computer‑literacy and competency assessment to ensure they can navigate our environment including electronic charting, shared understanding of company values, and use of our web‑based communication application. Compensation and Benefits

Wage range for Occupational Therapist: $ – $. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits, and 401(k) (up to 4% match).
