What you will do

The Behavioral Health Resources Coordinator will serve as an advocate and point of contact for patients needing behavioral health services. The BHRC will assist patient with navigating medical systems and available resources within SHC and the community. The BHRC identifies barriers to treatment, assists with access to appropriate resources, resolves issues that impact care progression, and improves patient experience. The BHRC will perform delegated functions of a case manager, social workers, and interdisciplinary team.

She/he will actively communicate with external agencies and resources to create a smooth transition of care. The focus will be on understanding the needs of the patient and facilitating the implementation of the recommendations of the treatment team.

Responsibilities
  • This role works in collaboration with the behavioral health care team.
  • Performs all duties and responsibilities in accordance with Stanford Medicine Partners’ policies and procedures, including its Service Standards and its Code of Conduct.
  • Coordinates care and access to SHC and SMP resources for patients needing behavioral health services to improve patient outcome and experience.
  • Provides coordination of care to behavioral health resources for patients who cannot be seen by SHC resources or associated therapy vendors due to insurance or access issues.
  • Reviews all unprocessed referrals to determine best resources for care coordination so patient is not left with an unresolved referral.
  • Verifies patient's health plan eligibility and benefits to identify in network resources.
  • Functions as a resource for patients to assist in understanding care team’s referral and the steps to be taken to access care.
  • Documents the plan for coordination with consideration for provider teams, individual, and family.
  • Identifies barriers to treatment and facilitates resolution of barriers with appropriate care teams/resources.
  • Participates in team meetings to improve quality of patient care.
  • Participates in department improvement projects.
  • Assists in creating a directory of resources for behavioral health patients.
  • Facilitates patient engagement and follow-up in behavioral health care.
  • Tracks referrals to completion and ensures accurate chart documentation and communication with the referring clinicians.
  • Assists with scheduling appointments as needed.
  • Adheres to safety, Population Health and Organizational goals, HIPAA and compliance policies.
  • Performs other duties as assigned and participates in organization projects as assigned.
Education Qualifications
  • Bachelor’s degree in a work-related discipline/field from an accredited college or university.
Experience Qualifications
  • Two (2) years of behavioral health related work experience.
Required Knowledge,

Skills and Abilities
  • Knowledge of Medical Record Documentation and EPIC.
  • Ability to assist Care Team in addressing physical, psychological, social and resource needs of the patient and family/caregiver and provide appropriate support as needed.
  • Ability to communicate with patients in distress and displaying challenging behaviors.
  • Ability to collaborate with other members of the interdisciplinary health care team to ensure quality care.
  • Ability to work with others in a flexible, cooperative and collaborative manner.
  • Ability to research and compile resources to assist patients in accessing care.
  • Ability to research and understand health plan guidelines and articulate to others.
  • Knowledge of computer systems and software used in functional area.
  • Ability to work with multiple referrals and patients concurrently.
  • Ability to follow through on all tasks and referrals to completion.
  • Ability to schedule appointments.
  • Ability to interact in a respectful, professional manner in accordance with SMP policies and procedures.
