About the Position
We are seeking a Registered Nurse – Case Manager to coordinate and manage patient care throughout hospitalization and post-discharge. The RN will assess patient needs, develop and implement care plans, collaborate with physicians, nurses, and social workers, ensure appropriate utilization of resources, and provide patient and family education to promote positive health outcomes.
Requirements
Requirements:
Current RN license in [State]
Minimum [2] years of nursing experience (acute care preferred)
Case management certification (CCM) preferred
Strong clinical assessment, care coordination, and communication skills
Knowledge of insurance, utilization review, and discharge planning processes
Ability to work independently and as part of a multidisciplinary team
